Next Steps: After you fill out this form, we will do our very best to follow up with you at the contact you provide within two weeks. We will proceed to a phone intake, followed by a short consultation period to make sure that the fit between clinician and patient is right for all involved. Please respond to our outreach within the week. If we do not hear back from you after a clincian has contacted, we will move down the waitlist and you will have to submit a new intake. Thank you.
